After completing the steps below (i.e. you have created your survey); you can view the format that will be displayed by the agent by clicking the " Survey Form" button.

Survey Questions

Under the "Survey Setup" tab -> "Survey Questions" tab, you can create your questions by pressing the "New" button.

Screen Order

To display questions sequentially in the survey, you must define a display order for each question.

Determining the Response Type

To specify the format of the answer, you can choose one of the following types.

• Optional [Radio Button]
• Optional [Combo Box]
• Optional [Multiple Check Boxes]
• Text Area
• Note Area
• Medium Script
• Subtitle

Creating Answer Options

You can add answer options by clicking the "Options" button of the relevant "Survey Question".

Start Script

Under the "Survey Setup" tab -> "Startup Script" tab, you can add an introductory speech.

Closing Script

Under "Survey Setup" tab -> "Closing Script" tab, you can add a closing statement.

View Survey Responses

All surveys for this Survey ID will be listed under the "Answers" tab.

You can view the document on which the survey was made by clicking on the " Source Document ID".

You can also view the survey by clicking the " Survey Form" button in the relevant line.
